Important Tips

1. To complete this application form, you are required to provide the following information:

  • Personal Information
  • Type of Program: Degree programs including B.Sc. (Bachelor of Science), M.D. (Doctor of Medicine), Pharm.D. (Doctor of Pharmacy), D.D.S. (Doctor of Dental Surgery), M.Sc. (Master of Science).
  • Level of Interest: B.Sc. (Bachelor of Science), M.D. (Doctor of Medicine), D.D.S. (Doctor of Dental Surgery), M.Sc. (Master of Science), 
  • Major (Program) of Interest
  • Supporting Documents

2. Supporting Documents: You need the below-mentioned documents in '.jpeg', '.pdf', '.zip' format.

  • Motivation Letter/Statement of Purpose (Mandatory for Graduate and Non-Degree Programs Applicants)
  • Résumé or C.V. (Mandatory for Graduate and Non-Degree Programs Applicants)
  • Academic Degrees and their Transcripts of Records for Degree Programs Applicants:
    Undergraduate applicants are required to provide us with Secondary School Certificate (SSC) and Higher Secondary School Certificate (HSSC), or their equals.
    Graduate applicants who are applying for M.Sc. level should provide us with B.Sc. degree, SSC, and HSSC along with their complete transcripts of records.
    Graduate applicants who are applying for Specialty Level should provide us with Secondary School Certificate (SSC), Higher Secondary School Certificate (HSSC) or their equals and M.B.B.S./M.D. degree or their equivalents along with its complete transcript of records. They should also provide their Certificate of Practice (COP)/Certificate of Internship.
    Graduate applicants who are applying for Subspecialty or Fellowship level should provide us with M.B.B.S./M.D. or their equivalents, and Specialty degrees along with their complete transcripts of records. They should also provide their Certificate of Practice (COP)/Certificate of Internship.
  • Academic Degrees and their Transcripts of Records for Non-Degree Programs Applicants (Post-Doctorate, Short-Term Course, Medical Elective, Summer Program, etc.):
     

3. You do not have to submit your application in one sitting, though it is a short application form with a few stages. You can save your information/uploaded documents at any stage and come back later to complete the rest of the application form for final submission.

4. Your application will not be successfully submitted unless you upload all the necessary documents marked with an asterisk (*) and also go through all stages and click the "Submit" button in the final stage. Please note that only submitted applications will be processed.

5. After you submit your application, you will receive an application code.  Please remember this code since it is required for future follow-ups.

6. A regular applicant with the above information and documents ready at hand needs at most 10 minutes to complete and submit the application.

After you successfully submit your completed form, your application and documents will be evaluated, and you will be contacted by NKUMS Office of International Admissions in case more documents are required.
